Learning Disabilities Joint Commissioning Manager


 

I work for Southampton City Primary Care Trust
I am responsible for planning and purchasing all health services for people with a learning disability and their carers.

  • The PCT funds Hampshire Partnership Trust to provide specialist learning disability services.
  • The PCT funds your local doctor and other community staff.
  • The PCT provides some community services itself.
  • The PCT funds the care you need if you go into hospital.
  • The PCT funds some care if your needs are complex and require intensive support, this is call Continuing Health Care.
  • The PCT gives money to the City Council for services which historically been provided by the NHS but are now the responsibility of local councils, such as day care and support for people who lived in hospitals

Here are some of the things I do.

  • I develop plans to ensure that people with a learning disability have the same chances of being healthy as other people in the community.

  • I meet up with people with a learning disability and family carers when asked to talk about their concerns.

  • I talk to other people such as doctors and nurses responsible for planning and providing health services for people in the City to make sure they include people with a learning disability and provide the right service.

  • I lead a project helping a number of people leave long stay hospital and move into their own home.

  • I tell Hampshire Partnership Trust what people living Southampton City need from learning disability services.

  • I write reports and keep the Primary Care Trust up to date on what is needed and how we can improve people’s health.
